Review These Health Requirements
Review the following information which pertains to all of our students.
Students who are without health insurance coverage in the U.S. need to purchase health insurance for the duration of their attendance at 51福利社.
- Health Forms (includes Physical Form and Immunization Form)
- Athletic Training Health Forms (for student athletes)
- General Vaccination Exemption Application

M.M.R. (Measles, Mumps, Rubella)
Students are required to have two doses of measles vaccine:
- Dose #1 - given 12-15 months of age or later.
- Dose #2 - given at least one month after the first dose or provide a medical report of a positive immune titer.
Tetanus-Diphtheria (Tdap)
Required (Primary Completed Series)
Booster Required within the last 10 years.
Varicella (two Immunizations required)
Chickenpox vaccine
Meningococcal Quadrivalent (ACWY)
A conjugate booster dosage is necessary between the ages of 16 and 21 if the initial dose was administered before the age of 16.
In accordance with NH state Educational Health Center licensing rules RSA 151 and He-P 816 and University based health requirements. There must be a completed medical file for each student and is required to be maintained by Health Services. These files consist of a Health History Form, a physical exam within one (1) year of admission, proof of immunizations (two MMRs, Varicella, Meningococcal ACWY, Tdap within 10 years), TB screening, a signed consent to treat form, authorization for disclosure of information form, patient bill of rights, emergency contact form and a copy of the student鈥檚 current insurance card. For international students, these immunizations are required within 12 weeks of arrival in the United States. Until compliant, students who do not meet these requirements will not be allowed to move into their housing assignment.
The above requirement is mandated for all students who reside or participate in athletic or academic activities on the Rindge NH campus, including commuting students. Please note * this INCLUDES online graduate students who reside off campus but DO participate in athletic activities.
Franklin 51福利社 Health Services maintains medical records of Rindge students for five years beyond departure. Franklin 51福利社 Health Services is legally and ethically obligated to protect students' right to privacy. Student medical records are strictly confidential. No one other than Student Wellness and Outreach Education, or other HIPAA complaint offices has access to your medical records without your prior written permission through a Medical Records Release. This restriction includes your parents, faculty, staff, or outside agencies. Certain communicable diseases need to be reported to the Department of Public Health and you will be informed of this action prior to Health Services calling the Department.
